"Cecum Ascends, Travels, Descends, Sigmoidly": large intestine regions

The large intestine frames the small intestine like a picture frame — following its path in order traces the exact shape it takes in the abdomen.

📖 Full Breakdown

Six regions, one continuous frame around the abdomen

No digestion happens here — only water absorption and stool formation.

Cecum
The blind pouch
Receives material from the ileum through the ileocecal valve. The appendix is attached here.
Ascending colon
Right side, going up
Travels up the right side of the abdomen to the hepatic flexure, near the liver.
Transverse colon
Crossing the abdomen
The most mobile segment, crossing horizontally from the right side to the left.
Descending colon
Left side, going down
Travels down the left side of the abdomen from the splenic flexure, near the spleen.
Sigmoid colon
S-shaped final stretch
The most common site of diverticulitis and colorectal cancer — a high-yield clinical fact tied directly to this specific region.
Rectum
Storage before elimination
Stores formed stool until it passes through the anal canal.
🩺 Clinical / Exam Application
A patient presents with left lower quadrant abdominal pain, fever, and a history of low-fiber diet. This presentation points strongly toward diverticulitis of the sigmoid colon — because the sigmoid is both the most common site for diverticula (small pouches) to form and the most common site for those pouches to become inflamed. Knowing which segment sits in the left lower quadrant lets you connect the pain location directly to the anatomy.
⚠️ Exam Alert
The hepatic flexure (near the liver, between ascending and transverse colon) and splenic flexure (near the spleen, between transverse and descending colon) are frequently tested landmarks — know which organ each flexure is named after and which side of the abdomen it sits on.
🚧 Common Trap
Don't assume the large intestine digests food the way the stomach and small intestine do. Its only major job is absorbing water and electrolytes and forming solid stool — there is no significant enzymatic digestion happening here.
✅ Quick Check
A patient has left lower quadrant pain. Which region of the large intestine is most likely involved, and what condition is classically associated with it?

Common Exam Questions

❓ What are the six regions of the large intestine in order?
✅ Cecum → Ascending colon → Transverse colon → Descending colon → Sigmoid colon → Rectum.
❓ Which region of the large intestine is most commonly affected by diverticulitis?
✅ The sigmoid colon — its S-shape and lower pressure zones make it the most common site for diverticula to form and become inflamed.
