Before We Start
Who needs bleeding precautions and why?
Bleeding precautions are implemented for patients at high risk for abnormal or excessive bleeding. The goal is to prevent injury that could trigger a bleed, and to catch early signs of bleeding before they escalate to a life-threatening hemorrhage.
Patients on anticoagulants
Warfarin (Coumadin), heparin, enoxaparin (Lovenox), apixaban (Eliquis), rivaroxaban (Xarelto), dabigatran (Pradaxa). These medications thin the blood intentionally — but they also make the patient vulnerable to excessive bleeding from minor trauma.
Patients with thrombocytopenia (low platelets)
Normal platelet count: 150,000–400,000/µL. Below 100,000 = thrombocytopenia (monitor closely). Below 50,000 = significant bleeding risk (implement precautions). Below 20,000 = severe risk (spontaneous bleeding can occur without any injury).
Causes: chemotherapy, bone marrow suppression, ITP (immune thrombocytopenic purpura), heparin-induced thrombocytopenia (HIT), DIC.
Patients with clotting factor deficiencies
Hemophilia A (factor VIII deficiency), Hemophilia B (factor IX deficiency), von Willebrand disease. These patients bleed excessively even from minor injuries because their blood cannot clot normally.

B — Bleeding Gums
Gum bleeding during oral care — an early and often overlooked sign
Bleeding gums in a patient with normal platelet counts and no anticoagulants might just be poor dental hygiene. But in a patient on anticoagulants or with thrombocytopenia, bleeding gums during routine oral care is a significant warning sign.
When to assess: During oral care — brushing teeth, oral suctioning, or simply opening the mouth for assessment.
What to look for: Blood in the mouth, on the toothbrush, or in oral secretions. Gums that bleed spontaneously without any contact.
Nursing action: Use a soft-bristle toothbrush or foam swabs for oral care. Avoid flossing. Document and report new onset gum bleeding. Do not use water piks.
💊 Switch to foam oral swabs for any patient on anticoagulants or with platelets below 50,000. Toothbrush bristles can cause gum trauma and bleeding.
E — Epistaxis
Nosebleeds — spontaneous or prolonged
Epistaxis means nosebleed. Everyone gets nosebleeds occasionally — dry air, nose blowing, minor trauma. In a patient at risk for bleeding, a nosebleed that is spontaneous (no obvious cause), prolonged (more than 10–15 minutes), or recurrent is significant.
First-line management:
• Sit patient upright, lean slightly forward (not back — swallowing blood causes nausea and vomiting)
• Pinch the soft part of the nose (not the bridge) continuously for 10–15 minutes
• Apply cold compress to the bridge of the nose
• Do NOT pack the nose without a physician order
When to escalate: Bleeding does not stop after 15–20 minutes of direct pressure, patient is hemodynamically unstable, or airway is compromised → call provider immediately.
💊 "Lean forward, not back" — leaning back allows blood to drain into the throat, causing swallowing of blood and potential aspiration. Always lean forward.
E — Ecchymosis
Bruising — especially in unusual locations or from minimal trauma
Ecchymosis is bruising — the discoloration of skin caused by blood leaking from damaged capillaries into surrounding tissue. In at-risk patients, bruising can occur from minimal trauma or even spontaneously.
What to look for:
• Bruising in unusual locations — places not typically prone to trauma (torso, inner arms, abdomen)
• Bruising disproportionate to the reported injury
• Bruising that is rapidly expanding
• Multiple bruises in various stages of healing (different colors indicate different ages)
Assess at every assessment: Check skin thoroughly during bathing and repositioning. Document size, location, and color. Note new bruises vs. known bruises.
Important distinction: Ecchymosis vs. hematoma. Ecchymosis is flat. A hematoma is a collection of blood that forms a raised, palpable mass — more serious, may require drainage.
💊 Use a body diagram to document bruise locations on admission. This protects the patient (abuse screening) and protects the nurse (pre-existing injury documentation).
P — Petechiae
Tiny red/purple dots that don't blanch — the most specific sign of low platelets
Petechiae are pinpoint-sized (1–3mm) red or purple spots on the skin caused by bleeding from tiny capillaries. They do NOT blanch (turn white) when you press on them — this distinguishes them from a rash, which does blanch.
Why they appear: When platelet counts are very low, even the minimal pressure of daily activities causes capillaries to leak. Petechiae typically appear first on dependent areas (lower extremities) and areas subject to pressure or friction.
Where to look:
• Lower legs and feet (most common location)
• Inner arms and antecubital fossa
• Areas where blood pressure cuff is applied
• Oral mucosa (mouth) and conjunctiva (inner eyelids)
Blanch test: Press a glass or clear plastic cover firmly on the spots. Petechiae remain red/purple. A rash blanches (turns white) under pressure.
💊 New petechiae on a patient receiving heparin = STOP and notify provider immediately. This could be HIT (heparin-induced thrombocytopenia) — a serious, potentially life-threatening complication.
Bleeding Precautions
What nursing actions reduce bleeding risk
Avoid invasive procedures when possible
Minimize injections, venipunctures, and IV starts. When necessary: apply pressure for at least 5 minutes after any puncture. Use the smallest gauge needle possible. Avoid IM injections — use subcutaneous or IV routes.
Protect from injury
Pad side rails if confused. Non-skid socks to prevent falls. Soft-bristle toothbrush or foam swabs. Electric razor only — no straight razors. Avoid rectal temperatures and suppositories. Avoid vigorous nose blowing.
Monitor laboratory values
INR/PT (for warfarin), aPTT (for heparin), platelet count, CBC. Know the therapeutic ranges and when to hold medications or notify the provider. Critical values require immediate notification.
Patient and family education
Teach: report any unusual bruising or bleeding immediately. Avoid aspirin and NSAIDs unless ordered. No contact sports or activities with high fall risk. Wear medical alert bracelet. Know signs of internal bleeding (blood in urine, stool, vomit).
🏥 Clinical Scenario — Warfarin Patient with New Findings
Mrs. Lopez, 68 years old, admitted for A-fib management. On warfarin with INR 3.8 (therapeutic range 2.0–3.0 — supratherapeutic). You are performing your morning assessment.
B
Bleeding gums: During oral care you notice blood-tinged saliva on the foam swab. Patient says "my gums have been sensitive lately." → New finding. Document time and appearance. Use gentler oral care technique.

Epistaxis: No nosebleed currently. Patient denies any. → No current concern but educate patient to report any nosebleed and to lean forward if one occurs.

Ecchymosis: During bathing you find a 4cm purple bruise on her left upper arm. Patient reports "I bumped it on the bed rail last night — I barely touched it." → Ecchymosis disproportionate to reported trauma. INR is supratherapeutic. Document size and location. Notify provider.
P
Petechiae: Several pinpoint red spots noted on bilateral lower legs. Press glass against them — they do not blanch. → Petechiae confirmed. New finding. Platelet count from morning labs: 89,000 (low — may be drug effect or HIT). Notify provider immediately.
→
Actions: Notify provider with all four findings. INR 3.8 + new ecchymosis and petechiae + low platelets = warfarin likely held, vitamin K considered, heparin held pending HIT workup. Implement full bleeding precautions. Document all findings with exact locations and times.

Critical NCLEX rules:
• Warfarin antidote = Vitamin K (phytonadione)
• Heparin antidote = Protamine sulfate
• Therapeutic INR for most indications = 2.0–3.0; mechanical valves = 2.5–3.5
• Therapeutic aPTT for heparin = 60–100 seconds (1.5–2.5× normal)
• Electric razor only — no straight razors
• Apply pressure minimum 5 minutes after any venipuncture
HIT (Heparin-Induced Thrombocytopenia): Platelet drop of 50%+ after starting heparin = STOP heparin immediately. Switch to a direct thrombin inhibitor (argatroban, bivalirudin). Paradoxically causes clotting, not just bleeding.
⚠️ The Trap — Normalizing Bruising in Elderly Patients
Elderly patients bruise more easily due to thinning skin and decreased subcutaneous tissue. This is normal aging. However, a medicated patient who bruises easily is not "just old" — their medication is contributing.
Never normalize bruising in a patient on anticoagulants or with low platelets. Every new bruise in these patients warrants assessment, documentation, and potentially a provider notification — especially if the bruise is in an unusual location, is expanding, or occurred from minimal trauma.
Also: new bruising + abdominal pain = possible internal hemorrhage. This is an emergency.
