SATA
Select All That Apply — treat each option as an independent True/False

SATA questions are the most feared on NCLEX — and the most misunderstood. The strategy that makes them manageable is simple: stop thinking of them as multiple choice. Each option is its own yes or no question.

Before We Start
Why SATA questions feel so hard

Traditional multiple choice questions have one right answer. You can use process of elimination, pick the best option from the choices given, and sometimes use test-taking tricks to narrow down options.

SATA questions break all of that. There could be 2 correct answers or 5. You get no partial credit — if you select 4 correct answers but miss 1, or accidentally select 1 wrong answer, the entire question is wrong. No partial credit on NCLEX.

The reason students struggle isn't that SATA is inherently harder — it's that students bring the wrong mental model to it. They try to find the "best" answers or the "most likely" answers. That's not how SATA works.

Treat every SATA option as an independent True/False question.

For each option, ask: "Is this statement true or false for THIS patient in THIS scenario?"

Select all the TRUE ones. That's it. Never look for a pattern, never pick a "safe" number, never compare options to each other.
The Four-Step Strategy
How to approach every SATA question
Step 1
Read the stem completely — know exactly what is being asked
The stem is the question itself. Read it all the way through before looking at the options. Identify:

• Who is the patient? What is their condition?
• What is the nurse doing? (Assessing, teaching, intervening, prioritizing?)
• What timeframe? (Immediately after surgery? 24 hours post-op? On discharge?)
• Are there any critical clues? (Recent medication change? Lab values? Vital signs?)

The stem gives you everything you need to evaluate the options. Never rush past it.
💊 Cover the options with your hand or finger. Read the stem. Know what you're looking for before you see the choices.
Step 2
Predict before you read — what would you expect to be correct?
Before reading the options, take 10 seconds and think: "For this patient, in this scenario, what would a safe nurse do? What findings would I expect? What teaching would I provide?"

This prevents the options from anchoring your thinking. Option writers are skilled at making wrong answers sound plausible — especially when you read them before you've thought about the question independently.

If you've predicted 2–3 things before reading, you have a framework to evaluate each option against.
💊 This works especially well for assessment and teaching questions. "What findings would I expect in a patient with congestive heart failure?" → crackles, edema, JVD, dyspnea — then confirm or deny each option.
Step 3
Evaluate each option independently as TRUE or FALSE
This is the core step. Cover all other options. Look at only one at a time. Ask: "Is this statement true for this patient?"

If TRUE: Select it. Move on.
If FALSE: Do not select it. Move on.
If UNSURE: Mark it tentatively. Come back after evaluating all others.

Critical rule: Do NOT compare options to each other. Do not think "well, option B is better than option A so I'll pick B." Each option stands completely alone — true or false, independent of all others.
💊 If you catch yourself thinking "I'll pick these three because they go together" or "option D can't be right if A is right" — STOP. You've slipped back into multiple-choice thinking.
Step 4
Re-read the stem after selecting — do your answers make clinical sense together?
After selecting all your TRUE options, do a final check: re-read the question stem and your selected answers together. Ask: "If I gave this patient this care, would it be safe and appropriate?"

This catches the occasional mistake where you selected something that's generally true but not true for THIS specific patient scenario.

Do not change your answer unless you have a specific, clinically sound reason to do so. First instincts on SATA are often correct — second-guessing based on anxiety is not a reason to change.
Common Traps
What makes wrong SATA options convincing
Trap 1 — The "generally true" option that doesn't apply HERE
Example: Question about a patient with hypokalemia. Option: "Administer calcium gluconate IV."
Calcium gluconate is given for hyperkalemia — not hypokalemia. It's a real medication used for a real electrolyte problem — just not THIS electrolyte problem. The option sounds clinical and reasonable but is wrong for this scenario.
Trap 2 — The option that's only partially true
Example: "Assess the patient's pain level every 4 hours." For a post-op patient in the immediate recovery period — pain should be assessed every 1–2 hours, not every 4. The action is correct (assess pain) but the frequency makes it wrong. Read EVERY word of each option.
Trap 3 — The option that requires physician orders
Example: "Increase the oxygen flow rate." Unless there is a provider order or a standing order that allows this, the nurse cannot independently increase oxygen. The correct independent nursing action is to notify the provider. Watch for options that have the nurse acting outside their scope without an order.
Trap 4 — Picking a "safe" number of options
Students often think "SATA usually has 3 right answers" or "I shouldn't pick all 6 — that seems like too many." There is no rule about how many correct answers there are. Some SATA questions have 2 correct answers. Some have 5. Select based on truth, not based on how many answers feel "right."
🏥 SATA Practice Question — Apply the Strategy
A nurse is caring for a patient with digoxin toxicity. The patient's digoxin level is 3.2 ng/mL (therapeutic 0.5–2.0). Which actions should the nurse take? Select all that apply.
A
"Administer the next scheduled dose of digoxin." → FALSE. Digoxin level is toxic. Hold the medication and notify provider. Never give more digoxin when the level is already toxic.
B
"Notify the physician of the lab result." → TRUE. A toxic digoxin level requires immediate provider notification.
C
"Place the patient on continuous cardiac monitoring." → TRUE. Digoxin toxicity causes dysrhythmias. Continuous EKG monitoring is appropriate.
D
"Check the patient's potassium level." → TRUE. Hypokalemia potentiates digoxin toxicity. Checking potassium is essential — it directly affects management.
E
"Administer calcium gluconate IV." → FALSE. Calcium gluconate is for hyperkalemia cardiac stabilization — not digoxin toxicity. The antidote for severe digoxin toxicity is Digibind (digoxin immune Fab).
Correct answers: B, C, D. Notice how A and E both sound clinical and reasonable but are wrong for this specific scenario. The SATA strategy of evaluating each option independently catches both.
📌 NCLEX Application
SATA questions appear throughout the NCLEX and are weighted heavily because they test higher-order thinking.

Content areas where SATA appears most often:
• Assessment findings for a specific condition
• Priority nursing interventions
• Patient education / discharge teaching
• Medications — side effects, contraindications, patient teaching
• Delegation — what can be delegated and to whom

The strategy that works every time:
Read stem → Predict → True/False each option independently → Re-read stem with selections → Submit

The more clinical knowledge you have, the easier SATA becomes. Content knowledge + True/False strategy = success.
⚠️ The Trap — Changing Your Answer Based on Anxiety
Research on NCLEX performance consistently shows that students who change their SATA answers change from correct to incorrect more often than the reverse.

Your first evaluation of each option is usually your best clinical judgment. Changing answers because "I have too many selected" or "this doesn't feel right" is anxiety talking — not clinical reasoning.

Trust your True/False evaluation. Only change an answer if you can identify a specific, clinical reason why your first assessment was wrong — not because of a vague feeling of unease.
