The dedicated Newborn Jaundice lesson covers the critical timing distinction — jaundice appearing after 24 hours (physiologic) versus within the first 24 hours (pathologic, an emergency). This lesson focuses on a different, equally important skill: how to actually perform the bedside physical assessment that detects jaundice and estimates its severity, plus the additional causes and treatment escalation that round out the full clinical picture.
💡 Why Visual Assessment Alone Can Mislead
Jaundice can be harder to visually detect in newborns with darker skin tones — this is exactly why the blanch test (pressing on the skin) and assessment of the sclera (whites of the eyes, which show yellowing regardless of skin tone) are both important techniques, not just a glance at overall skin color. Relying on skin tone alone risks under-recognizing jaundice in some newborns.

The Blanch Test
Press over a bony prominence to reveal underlying yellow color
Gently press a finger over a bony prominence (commonly the forehead, nose, or sternum) and release — this momentarily blanches (whitens) the skin, making any underlying yellow discoloration more visible against the temporarily lightened skin than it would be against normal skin tone alone. This technique is particularly useful for detecting mild or early jaundice, and for assessment across different skin tones.
Kramer's Zones — Head-to-Toe Progression
Jaundice progresses in a predictable pattern as bilirubin rises
Jaundice classically progresses in a head-to-toe (cephalocaudal) pattern as bilirubin levels rise — appearing first on the face and head, then progressing down through the trunk, and finally to the extremities (including palms and soles) as levels climb higher. This progression pattern (known clinically as Kramer's zones) gives a rough, visual estimate of severity at the bedside: jaundice confined to the face suggests a lower bilirubin level than jaundice that has progressed all the way to the hands and feet.
Important limitation: Kramer's zones give a general visual estimate, not a precise number — actual bilirubin levels (via blood test or transcutaneous bilirubinometer) are still needed to guide treatment decisions. The visual progression is a useful bedside screening tool, not a replacement for lab confirmation.

G6PD deficiency — a genetic cause beyond Rh/ABO and infection
G6PD Deficiency
A genetic enzyme deficiency causing increased red blood cell fragility
Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a genetic condition (X-linked, more common in males and in certain ethnic populations) in which red blood cells are more fragile and prone to hemolysis (breakdown), particularly when exposed to certain triggers — infection, certain medications, or even certain foods later in life. In a newborn, this increased hemolysis produces excess bilirubin, contributing to jaundice. This rounds out the causes of pathologic jaundice beyond Rh/ABO incompatibility and infection, and is worth screening for, particularly in populations where G6PD deficiency is more prevalent.
Escalation and Monitoring
When phototherapy isn't enough, and what to watch during treatment
Exchange Transfusion
Reserved for severe cases unresponsive to phototherapy
In severe hyperbilirubinemia that doesn't respond adequately to phototherapy, or when bilirubin levels approach the threshold for kernicterus risk, an exchange transfusion may be performed — this procedure removes the newborn's blood (and its high bilirubin/antibody load) in small increments while replacing it with donor blood, directly and rapidly lowering bilirubin levels rather than relying on the slower conversion process of phototherapy.
Additional Phototherapy Monitoring
Temperature and stool changes to watch for during treatment
Temperature monitoring: Phototherapy lights generate heat, creating a risk of hyperthermia — temperature should be monitored regularly during treatment, distinct from the general newborn hypothermia risk covered in the Newborn Assessment lesson.
Loose, green stools: An expected, normal finding during phototherapy — this reflects the bilirubin being actively excreted through the stool as intended, not a sign of illness or intolerance, and shouldn't be mistaken for diarrhea requiring separate workup.
🏥 Clinical Scenario — Using the Assessment Tools Together
A newborn, now 72 hours old, is being assessed for jaundice during a routine check.
Physical Exam
Blanch test over the sternum reveals yellow discoloration. Visual inspection shows jaundice on the face and upper chest, but the extremities remain a normal color. This progression pattern (face and upper trunk, not yet reaching the extremities) suggests a moderate level using Kramer's zones as a rough visual guide — reassuring relative to a case that had already progressed to the hands and feet, but still warrants an actual bilirubin level to confirm and guide any treatment decision.
During Phototherapy
The bilirubin level supports starting phototherapy. During treatment, the nurse notes loose, greenish stools and a slightly elevated temperature. The loose green stools are an expected, reassuring sign that bilirubin is being actively excreted — not a concerning GI finding. The elevated temperature should be monitored and addressed per protocol, since phototherapy lights themselves can contribute to hyperthermia.

⚠️ The Trap — Treating Loose Green Stools as a Concerning Side Effect
A nurse unfamiliar with phototherapy's expected effects might see loose, green stools developing during treatment and interpret this as diarrhea or GI intolerance requiring separate evaluation — potentially causing unnecessary alarm or workup. In reality, this stool change is a normal, expected, and even reassuring sign that the treatment is working as intended: bilirubin is being actively excreted through the GI tract.
The safeguard: Recognize loose, green stools during phototherapy as an expected finding reflecting successful bilirubin excretion, and communicate this reassurance clearly to parents who may otherwise become alarmed by the change in their newborn's stool.
