Apply the Fine-vs-Coarse Distinction
A longstanding, mild (fine) tremor is consistent with a normal, expected lithium side effect and doesn't by itself indicate a problem. But a NEW change — the tremor becoming noticeably worse, combined with new balance difficulty (ataxia) — represents a meaningful shift toward the SALT toxicity pattern, not just a continuation of the expected baseline side effect.
Investigate for a Trigger
The nurse asks about recent illness, hydration, diet changes, and any new medications — specifically screening for the known toxicity triggers (dehydration, sodium changes, NSAID or diuretic use).
