Children are not small adults — and nowhere is that more true than in their vital signs

A vital sign reading that would be alarming in an adult can be completely normal in a child, and vice versa. Getting age-specific ranges right is foundational to every other pediatric assessment skill.

Why pediatric vital signs can't be judged against adult norms

A heart rate of 130 would prompt real concern in an adult — but in a newborn, it's completely normal. Children's vital signs shift predictably with age: heart rate and respiratory rate start high in infancy and gradually decline toward adult ranges by adolescence, while blood pressure moves in the opposite direction, starting low and rising with age. Knowing these age-based ranges is the foundation every other pediatric assessment builds on.

💡 The Single Most Important Clinical Takeaway
Children compensate for shock by increasing heart rate — tachycardia is the EARLY warning sign. Blood pressure drops LATE, only once compensatory mechanisms are exhausted. A nurse waiting for hypotension to suspect shock in a child is waiting for a genuinely late, dangerous sign.

🏥 Clinical Scenario — Interpreting a Heart Rate Correctly by Age
A 4-month-old infant has a heart rate of 145 and appears otherwise well — feeding normally, active, good color.
Apply the Age-Specific Range
145 falls within the normal infant range of 100–160. Without knowing this specific age-based range, a nurse trained primarily on adult norms might mistakenly flag this heart rate as concerning tachycardia, when it's actually well within normal limits for this age.
Contrast With a Concerning Scenario
If this same infant's heart rate climbed to 180 alongside signs of poor feeding, lethargy, or delayed capillary refill, that combination — genuinely outside the normal range plus other concerning findings — would warrant a very different level of concern. The number alone isn't the whole picture; it needs to be read against both the age-specific range and the broader clinical context.

Vital sign questions test both range recall and shock recognition:

Range application: "Which heart rate would be considered within normal limits for a healthy 2-year-old toddler?" → A rate within 90–150.

Shock recognition: "A child in early shock is being assessed. Which vital sign change would the nurse expect to see FIRST?" → Tachycardia — blood pressure remains normal until shock is more advanced.

Late sign recognition: "A child's blood pressure has now dropped. What does this indicate about their shock status?" → Decompensated shock — a genuine emergency, since hypotension is a late finding in pediatric shock.
⚠️ The Trap — Reassurance From a Normal Blood Pressure in a Sick-Looking Child
Because blood pressure is such a heavily emphasized vital sign in adult care, it's tempting to use a normal BP reading as reassurance that a child isn't in shock. But given how late BP drops in pediatric shock, a normal blood pressure in a child with other concerning signs (tachycardia, delayed cap refill, mottling, irritability) should NOT be treated as reassuring — it may simply mean decompensation hasn't happened yet.

The safeguard: Weight tachycardia and other early compensatory signs heavily in pediatric assessment, and don't let a still-normal blood pressure delay escalation when other signs point toward shock.
