Two Hormones, Opposite Signals
Leptin
The Fullness Hormone
Leptin is produced by fat cells (adipose tissue) and signals satiety to the brain — generally, the more body fat a person has, the more leptin circulates, which should theoretically reduce appetite and increase energy expenditure as a feedback mechanism.
Ghrelin
The Hunger Hormone
Ghrelin is produced primarily by the stomach and signals hunger to the brain. Ghrelin levels typically rise before meals (stimulating the desire to eat) and fall after eating, making it one of the primary drivers of meal-to-meal hunger signaling.
Leptin Resistance
When The Signal Stops Working
In obesity, despite having higher fat mass and correspondingly higher leptin levels, the brain can become less responsive to leptin's signal over time — a state called leptin resistance. This means the fullness signal is technically present in the blood, but the brain doesn't respond to it as it should, undermining the expected appetite-reducing effect.

Leptin resistance is one reason obesity is not simply explained by a lack of willpower or awareness of fullness signals — the biological feedback system that's supposed to naturally regulate appetite and body weight can become impaired, making weight management a more complex physiological challenge than 'just eat less' implies.
