Diet, Surgery, and Medications
Beyond pernicious anemia, B12 deficiency can result from a strictly vegan diet (since B12 occurs almost exclusively in animal products), gastric bypass surgery (which removes or bypasses the parietal cells that produce intrinsic factor), or long-term use of certain medications that reduce stomach acid, which is needed to release B12 from food proteins.

Vegan patient's cause

Gastric bypass patient's cause
The surgery removed or bypassed the stomach cells that produce intrinsic factor, so B12 can't be absorbed even with adequate intake.

Since absorption itself is impaired, this patient often requires B12 injections or high-dose sublingual/nasal forms that bypass the normal intestinal absorption pathway, rather than standard oral supplementation alone.
